Using this guide

One place for the practical side of care.

Starting with a new practice can feel like a lot of moving pieces — paperwork, records, medication questions, and knowing who to call. This guide gathers the practical details in one place so you can focus on the visit itself. It’s written for new patients, returning patients, and the family members or caregivers helping someone get started.

Icare schedules visits and answers questions by phone. There’s no online booking, web intake, or portal signup to manage — call 702-988-3177 and the team walks you through each step, including which documents apply to your situation and how they’re completed.

  1. 01

Call 702-988-3177 and share, in your own words, the kind of support you’re looking for.

  2. 02

The team gathers basic contact, eligibility, and scheduling information and answers your questions.

  3. 03

Registration paperwork is handled by phone, in person, or through an approved secure channel — the team explains exactly what applies.

  4. 04

You receive the visit details, directions to 2470 E Flamingo Road, Suite B, Las Vegas, NV 89121, and guidance on how to prepare.

Before the visit

Prepare the information your clinician may need.

Appointment length, frequency, forms, and preparation can vary by service. The team will tell you what applies when scheduling — this list covers what helps most often.

  • 01

Photo ID and insurance card, if you have them

  • 02

A current list of medicines and doses, plus allergies, vitamins, and supplements

  • 03

The name and location of your preferred pharmacy

  • 04

Recent hospital, laboratory, imaging, or specialist records when available

  • 05

Guardianship or legal documents when applicable

  • 06

Your questions and the main goals you want the visit to cover

The first visit

What a first appointment may look like.

Every visit is shaped by the service and your needs, so details vary. Most first appointments, though, follow a familiar rhythm.

  1. 01

Check in with the front team, confirm your information, and finish any remaining paperwork.

  2. 02

Your clinician reviews the concern that brought you in, along with relevant history, medicines, safety, and daily functioning.

  3. 03

You talk through what matters most to you and what a realistic next step could look like.

  4. 04

Together you agree on a plan, which may include follow-up visits, testing, therapy, medication, recovery support, or coordination with another provider, depending on the evaluation.

Select any prompts you would like on your card. You do not need to share any personal information to use this guide.

This is not an emergency channel. For a medical emergency or immediate danger, call 911. For suicide or emotional crisis support, call or text 988.

During and after care

Treatment planning should be understandable.

Care doesn’t end when the visit does. These are the pathways patients ask about most between appointments.

01

Medication questions

Use the approved clinical channel for medication questions, and request routine refills before running out.

02

Insurance questions

Verification does not guarantee payment; final responsibility depends on the plan and the services provided.

03

Privacy & records

Use the approved records and communication pathways for protected information.

Common questions

Quick answers before you arrive.

How do I schedule or change an appointment?

All scheduling is handled by phone. Call 702-988-3177 to book, reschedule, or cancel, and give as much notice as you can when plans change.

Can a family member or caregiver help me get started?

Yes. Someone you trust can call with you or help gather information. What Icare can share with them about your care depends on your written permission and applicable privacy law.

Do I have to complete forms online?

No. Icare doesn’t use online forms or web intake. Paperwork is handled by phone, in person, or through an approved secure channel the team describes when you call.

Is telehealth an option for my visit?

Some visits may be available by telehealth when it’s clinically appropriate and eligibility requirements are met. Ask when scheduling, and the team can confirm whether your visit qualifies.
