A care plan only works when daily life allows a person to follow it. Missed rides, confusing paperwork, lapsed benefits, unstable housing, or a stack of unexplained referrals can undo good clinical work — not because someone isn’t trying, but because the system around them is hard to navigate. Coordination and recovery support exist to reduce that friction: when available, an Icare team member can help you understand what has been recommended, keep track of referrals and records, and connect you with community organizations that address practical needs.

Choice and independence

Outside resources remain separate from clinical care.

Recovery residences and community organizations may be independent entities. A listing or referral does not guarantee availability, eligibility, quality, coverage, or placement, and clinical care is not conditioned on using a specific outside provider.

Do I have to use the resources Icare suggests?

No. Suggestions are options, not conditions. Your clinical care at Icare is not conditioned on using any specific outside provider, residence, or program, and you remain free to choose.

Can Icare share my records with another provider or program?

Records are shared only with appropriate written authorization. You decide what is shared and with whom, and you can change your mind about an authorization you have given.

Is recovery support only for people in substance-use treatment?

No. Coordination can help anyone whose care spans multiple providers or practical barriers — medical, mental health, or recovery-related — depending on availability and clinical appropriateness.
