Why coverage questions rarely have one-word answers.

Every health plan sets its own rules. What a plan pays can depend on the specific service, the diagnosis, whether a deductible has been met, copay and coinsurance terms, network status, and whether a referral or prior authorization is required. Two people receiving the same care can have very different benefit results.

That is why Icare checks benefits person by person and service by service rather than publishing a plan list, which could quickly become outdated or misleading. Calling before your visit gives the team time to look into your specific situation and explain what your plan reports.

Insurance verification

Have the right information ready.

Verification happens by phone. Call 702-988-3177 with the patient’s full name, date of birth, insurance details, and the service being requested. Having these items nearby makes the call quicker and the benefit check more accurate:

What happens after you call.

A benefit check is a research step, not a promise. Here is the general path a verification request follows:

    The team checks available eligibility and benefit information with your plan when possible.

  3. 03

    You hear back about what the plan reported, including any referral or prior-authorization requirement that was identified.

    If an authorization is needed, the team can help start that process when appropriate.

  5. 05

    After your visit, your plan processes the claim and makes the final determination of coverage and patient responsibility.

Self-pay & uninsured patients

Ask about expected charges before scheduled care.

Patients who are uninsured or who choose not to use insurance may have rights under federal law to receive a Good Faith Estimate for scheduled non-emergency services. The estimate describes expected charges in writing before care begins, and there is a dispute process if a later bill is substantially higher than the estimate. Contact Icare for current self-pay information and an estimate when applicable.

Does a benefit check guarantee my care will be covered?

No. Verification helps you plan, but the final decision belongs to your health plan after the claim is processed. Benefit information reported before a visit can differ from how the claim is ultimately handled.

What if my insurance changes before my visit?

Tell the team as soon as you know. A new plan can have different rules for the same service, and re-checking benefits before the visit helps avoid surprises.

Do I need a referral or prior authorization?

Some plans require one for certain services and others do not. The team can help identify requirements during verification, though your plan makes the final decision.

Why can’t Icare quote an exact amount in advance?

For insured patients, what you owe depends on how your plan processes the claim, including deductible and coinsurance status at that time. Self-pay patients can request a Good Faith Estimate for scheduled care.

What if I receive a bill I don’t understand?
